Vitamin C (ascorbic acid) is a potent antioxidant, an essential cofactor for collagen synthesis and a critical immune supporter. Oral vitamin C cannot exceed a certain plasma threshold due to gut absorption limits. IV high-dose vitamin C, by contrast, reaches much higher plasma concentrations and acts at a pharmacological level.
Oral vs. IV Vitamin C
In the intestinal mucosa, SVCT1 and SVCT2 transporters quickly saturate at oral doses above 200 mg/day; the bulk of gram-level oral doses is excreted unabsorbed. The IV route bypasses this transporter limit — plasma vitamin C can reach 30–70× the level achievable orally. This high concentration opens a different biochemistry inside the cell.
High-dose IV vitamin C can act as a pro-oxidant in conditions of marked oxidative stress (intense exercise, chronic inflammation, cancer treatment, infection) — selectively targeting damaged or abnormal cells while sparing healthy ones. At normal doses, the antioxidant effect dominates.
Clinical Uses
Immune support: directly influences neutrophil and lymphocyte function; strengthens first-line defence. Used prophylactically during flu/cold seasons or in patients with recurrent URTIs.
Collagen synthesis: prolyl and lysyl hydroxylase enzymes depend on vitamin C; without it, tropocollagen cannot stabilise. High-dose IV vitamin C produces measurable effects on skin firmness, joint/cartilage health and wound healing.
Oxidative stress: neutralising free radicals broadens cardiovascular, neurological and chronic-disease-prevention effects.
Fatigue and recovery: the adrenal glands hold the body's highest vitamin C concentration; stress response and cortisol synthesis depend on it. Adrenal-support IV vitamin C is widely used during chronic stress.
Safety and Pre-Treatment Assessment
G6PD (glucose-6-phosphate dehydrogenase) deficiency screening is mandatory before high-dose IV vitamin C — deficiency can cause haemolytic anaemia. In patients with a history of kidney stones, the protocol is individualised to limit oxalate load and ensure hydration. Diabetic patients are informed about possible glucose-meter interference.
