Targeted Lipolysis (Enzymatic Lipolysis)

Dr. Metin Demir  ·  6 min read

Stubborn fat zones that resist diet and exercise are genetically programmed fat reserves and are hard to target with calorie restriction alone. Enzymatic lipolysis uses approved injection compounds to selectively dissolve fat cells in these zones for non-surgical contouring.

What Is Enzymatic Lipolysis?

Enzymatic lipolysis delivers lipid-disrupting compounds directly into subcutaneous adipose tissue. The most common agent is a bile-salt-derived phosphatidylcholine/deoxycholate combination. Deoxycholate disrupts adipocyte membranes, releasing cell contents; free fatty acids and glycerol are then cleared through the lymphatic system and metabolised in the liver.

Destroyed fat cells do not regenerate. The reduction is therefore lasting in the treated area — though remaining fat cells can enlarge with poor lifestyle habits, so healthy diet and exercise should continue.

Target Areas

Submental fat (double chin) is the most extensively studied indication. Upper arms, hip sides, inner thighs, lower abdomen and flanks are other frequent areas. Back fat — especially under the bra-line and above the waist — also responds well.

Procedure

The target zone is mapped; dose is calculated by fat-mass volume. Under local anaesthesia or cooling, multiple fine-needle injections are placed. Session: 30–60 minutes. Swelling, warmth and tenderness for 2–5 days post-session are normal and reflect the mechanism of action.

Optimal results usually require 2–4 sessions, 4–6 weeks apart. The interval allows the body to metabolise dissolved fat and the area to settle.

Important Notes and Eligibility

Lipolysis is a contouring procedure, not a weight-loss method. Ideal for patients near healthy weight with stubborn localised fat. Contraindications: pregnancy, breastfeeding, autoimmune disease, active infection, anticoagulant use. Detailed examination and eligibility assessment are mandatory before treatment.
